The HA is an annual requirement of the Move. program. You must retake the HA by June 30, 2014 to continue to be eligible for the Move. incentive through the end of this year.

Q: What is Be Colorado?
A:

The Be Colorado wellness program is an integrated health and wellness benefit provided by the CU Health Plan. Be Colorado serves as the comprehensive wellness component of the Plan, aiming to provide participants the tools, resources, environment, and motivation needed to enhance wellness. 

Q: Who is eligible for the Be Colorado online SUCCEED health assessment?
A:

University of Colorado, University of Colorado Health, and University Physicians, Inc. employees and retirees who are the primary member of a CU Health Plan are eligible. 

Q: Are covered dependent spouses and children eligible?
A:

Dependent spouses, including dependent spouses who are also employees of the University of Colorado, University of Colorado Health, University Physicians, Inc., and children are not eligible at this time. 

Q: Will my employer see my personal information?
A:

Your employer will not see anyone’s private health information. This would violate the Health Insurance Portability and Accountability Act (HIPAA), a Federal law. Your employer will only receive de-identified, aggregate reports. 

Q: How long does it take to complete the SUCCEED health assessment?
A:

The health assessment questionnaire should take 15-20 minutes to complete. If you are interrupted in the middle, you can save your answers and come back to it.

Q: What type of report will I get from the SUCCEED health assessment?
A:

As soon as you submit the completed assessment, you will receive an individualized plan that is based on your answers to the questionnaire. The more complete and honest your answers, the more helpful and complete your plan will be.

Q: I participated in a biometric screening. Will the results from this screening appear on my health assessment?
A:

Results collected from your biometric screening are confidentially uploaded to your health assessment to create a more convenient and customized health evaluation experience.
